Help Me Grow Medicaid
The Targeted Case Management (TCM) within Help Me Grow (HMG) Home Visiting has been developed to include federal Medicaid matching funds for specific services for Help Me Grow Home Visiting. The reimbursable TCM services are:
- Comprehensive assessment and periodic reassessment of individuals needs to determine the need for any medical, educational, social or other services
- Development and periodic revision of a care plan that is based on the information collected through the assessment
- Referral to needed services and supports within the community and related activities
- Monitoring and follow up activities
The planned implementation date for providers to begin billing Medicaid for TCM for HMG Home Visiting is July 1, 2012.

TCM within HMG Home Visiting is coordinated jointly between the Ohio Department of Job and Family Services (ODJFS) and the Ohio Department of Health (ODH).
